That's right. Nearly no one is without an health insurance. You have to have one. The costs for it depends on your status (simple said: your salary). If e.g. you're a student or unemployed you pay less or the state pays for you (but now it gets a bit complicated...so skip! XD).
Even homeless people without an insurance can go to see a doctor. It only they mostly don't want to.
No one is forced to go for a check-up.

So Paul could go to a doctor...but maybe he don't want to or simply ignore all advices.

Bei einem Facharzt muss man aber oft etwas zahlen. Wahrscheinlich ist das in dem Fall auch so, irgend eine besondere Art von Magenspiegelung oder so...

Oh, I didn't notice the discussion here. Paul isn't a student and have to pay for the good medical examination, which doesn't include Paul's cheap insurance. He did see a doctor (as you can read in this page), but abandoned the examination, because it costs money for him.
Jonouchi, I've paid for dental care, when I was in 2004 as a student in Germany. Because it wasn't include in my german insurance. Well, it was 14 years ago. May be something has changed during these 14 years, but I had this unfortunate experience. =(

No, it depends on the treatment. When a treatment is vital to keeping someone healthy or alive the insurance covers it, however if a treatment hasn't been proven to be effective or more effective than an already existing, cheaper treatment the patient can opt to pay for it themselves. The system isn't perfect of course, it might take a long time to list new treatments in the insurance catalogue, medicines that are beyond their patent phase rarely get approved for new uses because it isn't profitable enough to invest in studies, some medicines are deemed a pure luxury, etc. but noone with an acute problem like persistent pain would be required to pay for exams or treatments deemed necessary to find out what is wrong and cure them.
Trouble is some specialists will promote additional or alternative treatments because they make good money from it, some of course will only make you aware of additional options because they might be more effective in your case or have less side-effects.

There is a small co-pay for hospital stays (like 10€/day), however it is limited to something like 300€/year, is billed after your stay and will be waived or returned for people below a certain income.

Dental care is a bit different but not completely. Again, anything acute will be covered (infections, cavities), but dental repair and replacement will only cover the most basic fillings and dentures. You can increase the amount they will pay by doing regular checkups so any problems will be diagnosed early.
However as an exchange student I don't think you'll get the exact same conditions as a citizen.

What Wantkitty says sounds very familiar for me. I am sure, I heard that before somewhere. So it shouldn't be this wrong (especially in the dental care °w°).

I heard, it's up to 20 and max 25 days, that you have to pay this 10€/day if you need to stay in a hospital. But it depends on the health insurance company? Is that a wrong info? My HIC told me that they have the maximum with 25 days. °_°y
